Job Summary:

Custom Health is seeking a Health Professional to join our team as the Georgia In-Home Technology Liaison for a project aimed at managing the delivery, pairing of devices, education, and engagement with Medicaid recipients. The In-Home Technology Liaison will be responsible for coordinating the logistics of delivering and setting up assistive technology devices in the residences of eligible Medicaid recipients, working closely with pharmacies and other healthcare providers.

The primary responsibilities include coordinating and ensuring the smooth delivery of devices to members' residences, providing the necessary education on device usage, and engaging with members to support their healthcare needs. The In-Home Technology Liaison will collaborate closely with multiple pharmacies and healthcare professionals to facilitate the successful implementation of the project. This is a unique opportunity to contribute to a project that will improve the lives of thousands of Medicaid recipients in Georgia.

Key Responsibilities:

1. Delivery and Logistics Management:

  • Coordinate the delivery and installation of assistive technology devices to the residences of Medicaid recipients.
  • Collaborate with 3-5 partner pharmacies to ensure efficient and timely delivery of devices.
  • Train pharmacies on the deployment process and assist in scaling up delivery capabilities to handle 60 patients per week.

2. Patient Education and Engagement:

  • Provide on-site education and training to Medicaid recipients on the use of assistive technology devices.
  • Address any questions or concerns from recipients and ensure their comfort and understanding of device usage.
  • Foster positive engagement and build rapport with recipients to encourage ongoing device utilization and adherence.

3. Reporting and Documentation:

  • Maintain accurate and detailed records of device deliveries, patient interactions, and any changes or issues reported during visits.
  • Submit regular reports to the Solutions Manager, documenting progress, challenges, and recommendations for process improvement.

4. Compliance and Training:

  • Ensure compliance with all regulations and guidelines set forth by the Georgia Medicaid program and in-home provider requirements.
  • Stay updated on the latest regulations and policies related to assistive technology and medication management.

5. Qualifications:

  • Experience in clinical/hospital/EMS environments, CPR certification and medical terminology course completion.
  • Strong understanding of various disease states, particularly those prevalent among Medicaid excellent communication and interpersonal skills to effectively engage with patients and healthcare providers.
  • Ability to handle varied residence conditions and adapt to different environments.
  • Highly organized with exceptional logistical management abilities.
  • Proficient in documenting patient interactions and maintaining accurate records.
  • Familiarity with assistive technology devices and medication management systems is a plus.
  • Experience working with Medicaid recipients or in a similar community-based healthcare setting is desirable.

6. Requirements

  • Valid Driver’s License
  • Vehicle for driving to locations in Georgia, statewide.
  • Vehicle Insurance
  • Willing and able to travel the state of Georgia.
  • 3+ years experience in clinical, hospital, EMS environment.
  • CPR certification and completion of a medical terminology course are a bonus.
